The Excelicare Health Information Exchange (HIE) allows multiple care providers and stakeholders in a community to share patient data in a secure and controlled environment. Data items from multiple sources are collected and exchanged based upon the HITSP, HL7 - Continuity of Care Document (CCD) or other standards. A semantic mapping framework enables discrete data elements to be harvested and contextualized for presentation in custom displays for clinical review. Discreet data elements may also be used in analytics and for demonstrating meaningful use. Excelicare enables providers to view and print out the Continuity of Care Document (CCD), giving them easy access to their patients’ clinical data in a clinically relevant view. All clinical data elements of the CCD are available for use and the CCD may be configured to show all sections or only some selected sections.
Excelicare’s unique dynamic data model based architecture together with its semantic mapper allows new content of any type to be accepted in the data repository as contextualized discreet data which is then made available for display and analytics. The Excelicare Semantic Mapper helps achieve interoperability by facilitating the identification, conversion and exchange of metadata.
